JOB TITLE: IP Job Coach

DEPARTMENT: 2001

WORK SITE: Varies

JOB STATUS: Full Time and Hourly, Position Type: Temp to Hire 

POSITION REPORTS TO: To Program Manager or Case Manager, as assigned.

SCHEDULE: Varies, half hour lunch, Monday to Friday, overtime must be pre-approved by the direct manager.

 

ESSENTIAL DUTIES:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties. The following are the necessary duties to be performed:

 

  • Receives direct supervision from the Case Manager.
  • Responsible for overseeing, guiding and training individuals with developmental disabilities (known as Consumers).
  • Keeps daily records of job coaching hours, travel time, and entry time.
  • Understands and supports the objectives outlined in each Consumer's Person-Centered Plan.
  • Prepares all necessary documentation in a neat, accurate, and timely manner.
  • Documents monthly worksite training data and job coaching reports.
  • Advocates for services and supports that improve the quality of life for Consumers.
  • Assists the Consumer maintain a clean and safe work area.
  • Conducts safety training, enforce safety rules, and organize periodic safety meetings and evacuation drills with Consumers.
  • Provides oral and written feedback to Consumers regarding their progress.
  • Communicate with the Case Manager about any concerns, issues, and progress of Consumers.
  • Conducts off-site skills training for Consumers.
  • Promptly reports any injuries or incidents involving Consumers or oneself.
  • Accurately documents Consumers' goal progress (case notes) in the Case Magic database on a daily basis.
  • Adheres to attendance requirements and arrives on time.
  • Follow up with Consumers on the work progress of their daily tasks.
  • Accurately document information required for billing purposes, including Consumer’s clock-in, clock-out, lunch time, on-site and off-site time.
  • Submits necessary safety reports by the end of each month, including safety meetings, drills, vehicle inspection checklists, and any other reports requested by the Case Manager.
  • Must be able to drive to various off-site locations.
  • Fosters positive relationships with employers and other stakeholders while representing Ability Counts, Inc. in a professional manner.
  • May be required to work at multiple locations throughout the day or week.

 

ADDITIONAL DUTIES:

  • Performs other duties as assigned.  
  • Transport consumers to and from offsite training sites.
  • Perform regular vehicle inspections.
  • May transport consumer(s) to industrial clinics.

EDUCATION/EXPERIENCE:

 

Previous work experience working with people with disabilities; High school diploma or GED. Proficient in reading, writing and basic math skills; proficient in Microsoft Word, Excel and Case Magic software programs. Excellent communication and interpersonal skills. Valid California driver’s license and driving record acceptable to insurance companies. CPR and First Aid certificate required.

 

SKILLS, KNOWLEDGE AND ABILITIES:

Ability to read, analyze, and interpret common, technical documents and safety rules; operating and maintenance instructions; reports; legal documents; ability to respond to inquiries and complaints from customers, and other stakeholders; represent the company in a professional manner to top management, public and the board of directors; speak effectively before groups. Knowledge of risk management to include safety rules and regulations, OSHA compliance. Skilled in problem solving matters that may arise and create solutions; basic mathematical skills and operations.

COMPENSATION: $18.00-$21.00/HR

The above range is the range that we in good faith believe is the range of possible compensation for this role at the time of this posting. We may ultimately pay more or less than the posted range. This range may be modified in the future.
